What Is a PPD TB Test and Why Is It Required?

A PPD TB test is a way to check if your body has ever been exposed to the bacteria that cause tuberculosis. An injection under the skin is done, and the area is checked a few days later to assess if you have a reaction and how your immune system responds. The test is required for healthcare workers and school employees because TB is contagious and a serious disease when left untreated. PPD testing complies with safety regulations and reduces the risk of transmission.

How the PPD Test Is Performed

During the TB testing for PPD, a healthcare provider injects purified protein derivative (PPD) beneath the skin of your forearm with a fine needle. It creates a small bump at the injection site. Don’t cover up or scratch the injection site. At your follow-up visit, the site will be checked for potential reactions. The healthcare provider will measure the bump and check if it's firm, to see if it indicates TB exposure.

Follow-Up Steps for a Positive TB Test

If your TB test is positive, it doesn’t necessarily mean you’re sick. A positive result proves your body has been exposed to TB bacteria at some point and needs more testing. The next step is a chest x-ray or a blood test to see if the TB infection is latent or active. A positive latent result means the TB bacteria are in your body, but you’re not symptomatic or contagious. Patients with an active TB result are usually given additional tests to confirm the result and then given a regimen of 4 medications.

Understanding Your Test Results

PPD test results are interpreted based on the size of induration at the injection site, not redness. One of our healthcare team members will measure if you have a reaction and assess if you’ve been exposed to TB. You have a positive result if the induration meets or exceeds a specific threshold for a latent TB infection, which requires follow-up testing with an interferon-gamma release assay (IGRA) or chest x-ray. Negative results suggest you have no detectable TB exposure.
